Dr. Leonard Matheson
A Career in Rehabilitation, Research, and Innovation
Dr. Leonard N. Matheson has devoted his career to understanding human functional ability and helping rehabilitation professionals connect clinical recovery with meaningful participation in work and life.
His work spans occupational rehabilitation, functional capacity evaluation, neurorehabilitation, medical research, teaching, and the development of functional assessment tools.
His career began with pioneering work in occupational rehabilitation at Rancho Los Amigos Medical Center in California and continued through research, clinical practice, and teaching at the Washington University School of Medicine in St. Louis.
Advancing Work Rehabilitation
During the 1970s, Dr. Matheson and his colleagues began developing approaches to rehabilitation that placed meaningful activity and work demands at the center of recovery.
At a time when rehabilitation often focused primarily on medical recovery, this work asked a different question:
What does a person need to be able to do to return to work?
This thinking contributed to the development of work hardening, work capacity evaluation, and eventually broader approaches to functional capacity evaluation.
Dr. Matheson's early work was collaborative. Occupational therapists, physicians, psychologists, and other rehabilitation professionals contributed to the development of the field.
Among his colleagues were Linda Dempster Ogden, Susan Isernhagen, and Dennis L. Hart, whose work alongside Len helped advance occupational rehabilitation and functional evaluation.
Functional Assessment
Dr. Matheson's career has included extensive development of functional assessment methods and protocols.
His work has addressed physical capacity, lifting, work-related activity, functional performance, and the relationship between a person's abilities and the demands of work.
He is associated with the development of several influential assessment tools and protocols, including the Spinal Function Sort, Hand Function Sort, and Multidimensional Task Ability Profile (MTAP), as well as the EPIC Lift Capacity approach.
The underlying philosophy is consistent:
Functional assessment should provide meaningful information about what a person can actually do.
Academic Career
Dr. Matheson spent fifteen years as an Associate Professor in Occupational Therapy and Neurology at the Washington University School of Medicine in St. Louis.
His academic work provided an opportunity to combine clinical experience with research and teaching, further developing approaches to occupational rehabilitation and functional assessment.
He has taught and collaborated with rehabilitation professionals across North America and internationally.
Current Focus: Cognitive Functional Capacity Evaluation and Neurorehabilitation
Dr. Matheson's work continues to evolve with the changing needs of rehabilitation.
One of his current areas of focus is Cognitive Functional Capacity Evaluation (CFCE) and work-oriented neurorehabilitation.
For someone recovering from traumatic brain injury, stroke, or another neurological condition, physical strength may tell only part of the story.
Attention, memory, executive function, problem solving, judgment, endurance, and the ability to manage complex tasks can all affect a person's ability to return to work.
Cognitive functional evaluation seeks to better understand these abilities in relation to meaningful activity and occupational demands.
This represents an important evolution of the same principle that has guided Dr. Matheson's work for decades:
Understand the person. Understand the task. Understand the demands of the work.
Service to Veterans and Military Rehabilitation
Service members and veterans have also been an important part of Dr. Matheson's professional work.
Over the course of his career, he has contributed to military and veteran rehabilitation through education, program evaluation, neurorehabilitation initiatives, and clinical consultation.
His work has included collaboration with military medical providers and organizations addressing traumatic brain injury, psychological health, recovery, and reintegration.
In 1992, Dr. Matheson received a Certificate of Appreciation from the United States Marine Corps recognizing his service during Operation Desert Shield and Operation Desert Storm.
His commitment to helping service members and veterans remains an important part of his professional legacy.
Recognition
Dr. Matheson's contributions to occupational rehabilitation have been recognized throughout his career.
In 2021, the American Occupational Therapy Association honored him with its Health Advocate Award, recognizing his contributions as a pioneer, educator, and policy advocate in work-oriented neurorehabilitation.

Dr. Matheson's work has never been limited to a single test or protocol.
